This in-office procedure surgically removes the buccal fat pad from the hollows of the cheeks. Located just below the cheekbones, this collection of fat is easily identified when the lips are pursed. Excising this particular fat reservoir is performed with local injections of numbing medication, requiring less than an hour to complete. This is a scarless procedure as the incision is done from inside the mouth.
This procedure addresses full cheeks and adds more definition to the jawline. Although the buccal fat pad is relatively small, its impact on the contours of the cheek and jawline are significant. Removing this fat instantly reduces lower facial fullness to permanently banish unwanted puffy cheeks. The buccal fat pad is extracted using an incision inside the cheek, allowing for a recovery with minimal visible signs of surgery. Consequently, most patients are able to resume their usual schedule one to two days after the procedure.
Swelling of the cheeks following this treatment is expected; however, this will subside significantly within three to five days and completely resolve within two to three weeks. Some patients also report mild discomfort for twenty-four to forty-eight hours following surgery. We, therefore, recommend that our patients take two tablets of Extra Strength Tylenol every four to six hours for pain control. Additionally, patients should follow a soft diet for the first few days after surgery to avoid disturbing the stitches inside the cheeks. There is also no need for suture removal since Dr. Khorasani uses self absorbing sutures for this procedure.
Immediately following the procedure, the cheeks and jawline will be significantly contoured; however, post-surgical swelling may obscure the final results. Consequently, maximal cheek and jawline sculpting may require up to three months to fully appreciate. This prolonged wait period is rewarded by a permanently defined cheek and jawline profile.
The masseter muscle and the overlying parotid gland are located just in front of the ears, extending from the cheek bones to the angle of the jaw. Enlargement of these muscles or glands may be attributable to genetics or overuse from habitual teeth grinding and chewing. Strategically placed injections of neuromodulators such as Dysport or BOTOX are used to immobilize and reduce the size of these structures without impacting your ability to chew or produce saliva.
Excessively full masseter muscles and parotid glands are often responsible for overly prominent or asymmetric jawlines. Patients seeking to narrow the shape of wide, square-like jawlines are excellent candidates for this injectable therapy. Years of chewing causes a gradual increase in the size of the masseter muscles. However, certain populations may be more prone to masseter enlargement called masseter hypertrophy. In particular, female athletes are prone to get a “square” shaped jawline due to grimacing while lifting heavy weights. Patients with habitual grinding, malocclusion such as an overbite or underbite, and TMJ may also suffer from masseter hypertrophy. Unilateral hypertrophy of the masseter muscles can also be seen with habitual gum chewers. Chronic parotid enlargement may be genetic and is seen in healthy individuals. The enlargement of this gland may also be seen in patients with excessive alcohol consumption, diabetes, HIV and in bulimic patients.
Aside from its efficacy, patients love masseter muscle and parotid gland slimming since this treatment is associated with zero downtime. As dictated by your own personal comfort, you may resume normal activities immediately after treatment. We do, however, recommend that you avoid massaging the treated area for at least four hours after treatment. Some patients endorse a slight weakness when chewing for the first few days after treatment; however, this typically resolves within one week. Professional woodwind players should avoid this procedure as they may not be able to play certain notes for a few weeks.
Dysport/BOTOX in any form requires multiple days to exert its effect on the treated area. Furthermore, the effect of this procedure contributes to the thinning of the muscle fibers and glandular tissue. As such, maximal jawline shaping may not be visible for up to 8 weeks after the procedure. Please note that, as with all injectable treatments, the contouring effects of masseter muscle and parotid gland slimming are temporary. Most patients will require a repeat session after 8 weeks. However, unlike a Dysport/BOTOX treatment of the forehead, most patients do not need to come in every six months. Once the muscle and gland are thinned out, it requires a few years before one goes back to baseline.

Dysport/BOTOX-induced modifications to the jawline are first visible within 8 weeks, but optimal shape changes are achieved 8 weeks after the second round of injections. Discomfort during the first few days after surgery is usually effectively managed with Extra Strength Tylenol every four to six hours, but we will also prescribe you a stronger pain medication that may be used if the Tylenol is insufficient. For the first five to seven days following the procedure, an elastic chin wrap will be worn to compress the treated area and ensure optimal cosmetic results. During this period, significant swelling and bruising of the area is expected; however, these side effects typically improve within five to seven days and resolve completely within two weeks. Consequently, as dictated by your personal comfort, you may resume normal activities five to seven days after surgery.

The procedure experience is dictated by the treatment modality used:
A buccal fat pad removal requires injecting numbing medication into the treatment area, a process which may sting for five to ten seconds. After administering the local anesthetic agent, you should not feel any discomfort; however, you may continue to sense pressure, tugging, or warmth as the surgical instruments manipulate the cheek.
Masseter muscle and parotid gland slimming is achieved with meticulously placed injections of a neuromodulator such as Dysport or BOTOX. Each of these injections may induce a pin-prick like sensation; however, this discomfort is brief and mild.
The buccal fat pad removal portion of the K-Slim procedure will feel exactly the same as the stand-alone treatment; however, the masseter muscle and parotid gland slimming component will be virtually painless, as the area will be numbed by the lidocaine injections used for the buccal fat pad removal.
Liposuction jawline augmentation is a more involved procedure, requiring the creation of small entry sites (ports) to allow the liposuction device to reach under the skin’s surface. Each 2 mm port site will be thoroughly numbed with an injectable anesthetic fluid to ensure maximum comfort. Following anesthetization, the liposuction cannula (a small, blunt tube) will be inserted under the skin to begin treatment. At this time, an anesthetic fluid is infused under the skin to homogenize, or breakdown, the fat. Using a precise level of suctioning power, Dr. Khorasani will extract all available fat from this area to eliminate the unwanted bulge in its entirety.
The length of the treatment is highly dependent on the chosen therapy. Buccal fat pad removal typically requires one hour to complete. Masseter muscle and parotid gland slimming is usually finished within 15 to 20 minutes. As a combination of buccal fat pad removal and masseter muscle and parotid gland slimming, K-Slim procedures last approximately ninety minutes. Finally, liposuction jawline augmentation encompasses approximately three hours.
